new to this...have no idea where to start

I have a new client that brings new challenges...

I need to build a form where users can enter their name, address, e-mail and maybe a couple other things. Some fields would be mandatory, some fileds wouldn't.

WHen they click "submit" everything would get written to a database that the client could download each night.

I could build the visual aspects of the form in either Dreamweaver or Flash, but from their I have no idea where to go.

My server does allow MySQL and is PHP4 enabled.

Any help is really appreciated.
