Create a Story

Hi
I have been to a site where, when you arrive at the URL, a form box pops up and you fill in details, followed by about a dozen further boxes. When you are finished, the page generates a story that fills in the details you supplied (eg names, objects) etc.

I would very much like to put one of these on my site - would anyone be able to help me as to how I would go about doing such a project?
